Operose Health is the UK’s largest provider of primary care practices, serving more than 640,000 patients from communities around 60 practices.
The company has developed a unique model of primary care delivery ‘at scale’, tackling increasing demand, increased cost and static budgets to deliver 5 times the capacity of a traditional GP practice.
Acquiring new practices both through short term contracts offered through procurement processes as well as working with traditional GP partners looking for additional support, the business continues to grow its practices, welcoming new patients, and through expanding into new geographic areas.

With primary care acting as the gateway to the NHS and with challenges across the country with limited access, we recognised Operose Health’s model could provide safe services, affordability for the taxpayer and increase access for communities.
The business hadn’t fully completed the integration of previous acquisitions, leaving it operating two models of care and inconsistent cultures, which were affecting staff retention.
With the business’ previous owner seeking to divest its UK-based assets, we worked with the management team to transfer the business back to UK ownership and to maintain confidence of commissioners across the country.
Since our investment
- Immediately upon acquisition, we undertook a wide-ranging review of the business empowering it to focus on its core product: primary care service provision.
- With hands-on support and expertise, we supported the management team to improve staffing within practices, reducing agency spend to invest in permanent staff and consequently improve access.
- We invested in a single aligned management team for the whole business, with a new Chief Executive in Samatha Kane and a skilled operational leadership team working together across the country while maintaining strong clinical leadership from professor Nick Harding.

Twenty20 Capital acquired the business at a challenging time: fresh from the coronavirus pandemic, the business was loss-making and had been struggling to attract and onboard sufficient frontline staff in the face of significant shortages of key professionals.
With many contracts now in a ‘mature’ state following transformation and a need to retain existing work as well as secure new contracts, we worked closely with the management team to ensure a good fit before completing the transaction in 2021.
Since our investment
- Within the first six months, we worked with the management team to deliver a sustainable operating model more suited to the needs of the mature contracts held by the business
- We supported the management team to implement a “healthcare first” approach to support teams, driving back-office costs to an industry-low of 8% with a scalable model fit for the future and prioritising high quality clinical services
- Our workforce expertise allowed us to support a refresh of the employee value proposition, recruitment and retention – delivering 95% vacancy fill rates, an industry high
- The business has doubled its overall profitability, facilitating responsible investment in new products and supporting health and care commissioners with new challenges
- Our expertise and support has enabled the management team to secure renewed contracts and significant extensions to existing contracts for the first time in its history
- With our expertise and further investment, we have returned the business to growth with new contracts commencing in April 2025 adding 3,200 new colleagues and more than £150m annual revenue
- With a renewed focus on responsibility through our hands-on support and commitment to ESG goals, the business has scored highly in social value submissions and reduced its carbon emissions by 55% per £m of revenue
